Notary Title Transfer

To get a REAL ID in Louisiana, you’ll need to bring: 

• One primary Identity document (e.g. a Valid Passport or Birth Certificate) 
• One proof of social security (e.g. Social Security Card or W-2) 
• Two proofs of Louisiana residency (e.g. Utility Bill, Lease Agreement) 

All documents must be originals- no copies. If your name on Birth Certificate does NOT match DL\ID bring legal proof of the name change. (e.g. Marriage Certificate)

Here’s what you’ll need to get started: 

• Must be at least 15 years old for learner’s permit. 
• Must be at least 16 years old and carried the Learner’s Permit for 180 days for the Intermediate License. 

Bring: 

• Original Birth Certificate
• Social Security Card
• Parent/Guardian with photo ID (Legal Domiciliary Papers or legal Guardian papers if applicable. 
• Sealed envelope from driver’s education course.

 If you’re no longer using a vehicle, you should cancel the plate to avoid fees:

• Visit FAST TAG TITLE LICENSE & NOTARY and we’ll handle the cancellation for you
• Bring your license plate and photo DL/ID 

  To transfer a title in Louisiana, you’ll need:

• The original vehicle title or MSO (Manufacturer’s Statement of Origin), signed by both buyer and seller.
• A Bill of Sale or dealers invoice. 
• Odometer Disclosure Statement. 
• Proof of Insurance.
• Valid photo DL/ID for buyers. 
• Payment for tax, title, and registration fees.

 Yes, Louisiana requires registration for off-road vehicles.

You’ll need:

• MSO (Manufacturer’s Statement of origin) or Title. 
• Bill of Sale or dealers invoice

If your license is lost or stolen:

• Visit Fast Tag Title & License with your Social Security Card or know your Social Security number.
• Proof of Insurance if you have a vehicle registered in your name (for Driver’s License only). 
• We’ll help you apply for a duplicate license.
• A small replacement fee applies. 
• We can issue a duplicate on the spot in most cases.

Tip: Make sure your current address is on file when requesting a duplicate. 

If your plate was lost or stolen:

• Bring your photo DL/ID.
• Bring your Insurance.
• File a police report (recommended for stolen plates)
• We can issue a replacement plate and registration sticker.

Important: Never drive without a plate- visit us as soon as possible to get a replacement.

To request a duplicate title:

• Bring your photo DL/ID
• Provide The VIN number or License Plate number.
• Complete a Duplicate Title Application (DPSMV 1799) 
• If there’s a lien on the vehicle, the lienholder must request the duplicate or provide a lien release.
